New patients only. Please call to schedule your appointment at (423) 390-0768. Please mention your Local Flavor Certificate at time of setting appointment. Merchant will abide by gift certificate state laws. No cash or credit back. Each certificate must be used in one visit. Not valid on tax or gratuity. Limit one certificate per visit. Does it expire?
